well, people do make fun of fat people and tell them they have problems quite often, so that's clearly not true. also, some people genuinely are too thin, to the point that they look gross and they are hurting their health. it's kind of assumed that fat people know that they are fat, but that people who are too thin (if they are genuinely anorexic or have some other eating disorder) might not realize that they are too thin, so friends who wouldn't point out a fat person's weight issues might comment on a thin friends perceived weight issues. saying someone looks too thin can be an expression of genuine concern or just a body type preference, not necessarily jealousy.
